A preliminary conference version of this work appeared in the proceedings of the ACM-SIAM Symposium on Discrete Algorithm (Feng et al. 2021), which contains only parts of this paper. In the current paper, all of the results in Sections 5 and 6 and Online Appendices C and D are new and there are several additional insights throughout. Moreover, the current paper presents all the proofs and technical details, most of which were missing in the early conference version.
